Appointment
Step 1: Click the “MAKE AN APPOINTMENT” button below.
Step 2: Click “Create a New Account” on the site, and follow the steps to create a new account.
Step 3: Click the “Request Care” button, answer the questions and submit.
Step 4: Upon email notification, click “My Surveys“, read, fill out, and sign the three forms/agreements assigned to you.

Office Hours: Mon – Sat 10am – 4pm, Central Time Zone
Fee: $120/session (50 minutes); $180/extended session (80 minutes); $1,000/twelve 50-min sessions. Does not accept health insurance.
